Return application form to: Amanda Heather, Meadowcroft Cottage, Manor Road, Gnosall, ST20 0RJ, 07929351774 or e-mail a signed copy to : membership@churcheatontennisclub.org.uk. Once Amanda has either received a cheque, cash payment or a confirmation e-mail that you have sent a BACS payment, a code for the new gate padlock on the court will be sent back to you.
